Differences in surgical techniques
Both techniques involve the same incision in the eyelid to create a double fold. In the case of upper lid sagging excision, a double fold is created with skin excision at the double fold line. There is no difference in downtime between the two techniques.

Which technique is more suitable is determined during the simulation.
Counseling Method
During the counseling session, we ask about the desired shape and width of the double fold and simulate the double fold by placing a bougie (a stick-like instrument with a thin tip) on the eyelid. If the desired double fold can be reproduced at this time, incisional double fold is indicated.
If the bilateral width feels narrower than the desired width, the bougie position is widened, but if there is still too much skin coverage to achieve the desired width, an upper eyelid sagging excision is indicated.
If the desired double width is achieved but the skin is covering the eyelashes, an upper eyelid sagging resection will also provide a beautiful double fold with the eyelash line visible.

There are more points to consider, such as the following. Therefore, it can be said that the surgeon's experience makes a difference compared to the incisional double-exposed eyelid without skin excision.
The skin to be removed for upper eyelid sagging excision is usually in the range of 1 to 3 millimeters in my case. In cases where I find it necessary to remove more than 3mm of skin, I recommend a sub-brow incision.
Since no one patient's wishes or eyelid conditions are the same, during the counseling session, I try to keep my mind fully occupied so that I can suggest the most appropriate surgical technique.
