Since the zygomatic bone is morphologically continuous between the body and the arch, this surgical technique is used to simultaneously recede and widen both in one lump.
The approach is made through two incisions, one in the mouth and one in the anterior ear. Surgery is performed from these two directions to remove bone and osteotomy.
The first step is osteotomy of the zygomatic body (partially maxilla) from inside the mouth.
During this osteotomy, the entire zygomatic bone is turned inward to decrease facial projection and width, but at the same time it is important to move it headward to prevent sagging of the cheeks.
Next, an osteotomy of the arch is performed anterior to the TMJ through an anterior ear incision.
These two osteotomies provide almost full-length mobility of the zygomatic bone.
Depending on the degree of overhang, the osteotomy in the body is shifted medially by about 4 to 6 mm, and is fixed with wire + titanium plate.
After the bone is fixed, the suborbital rim is carefully shaved to eliminate the step with the surrounding bone.

Postoperative risks and side effects

- - - - - - - - - - - - - - - - - - - - - - - Infection (to fester): If there is increased or prolonged heat, pain, or swelling after surgery, infection is suspected. If infection occurs, oral medication, antibiotics, and cleaning are administered.
In severe cases of infection, the skin of the mouth and gills must be cut open to drain the pus.

-Blood Blood pools.: If blood accumulates in the wound after surgery due to bleeding, it will swell to a purple color from the chin to the neck. If the blood is left to accumulate, it can become infected and form a lump, so it must be treated as soon as possible.
