Do you remove the implantation threads for an incisional double fold?

Hello. My name is Kuroda, Deputy Director.

Many people ask, "When I have an incisional double fold, do you remove the threads from my previous implantation?" We are often asked this question.

In response to your question, we answer, "We try to take as much as possible. However, it is sometimes difficult to extract all of them.

The reasons for this and what it has to do with the implantation threads will be explained.

Reasons why thread removal is difficult

The reason why it is difficult to remove all the threads from the implantation technique is that there are many variations of the implantation technique.

There are various ways to thread the threads for implantation,

Backstop with the ball of thread on the conjunctival side

makes complete removal of the implant difficult.

Do you have an image that removing an implanted thread is as easy as pulling on the thread?
The thread ball of the implantation thread is adhered to the surrounding tissue and cannot be pulled out forcefully.
But once the thread ball is removed from the surrounding tissue, the embedded threads can be pulled out smoothly.

So-called uradome is a method that has become popular in recent years, especially among major clinics, because of its advantage that the thread ball does not show through the skin. As may be explained when undergoing backdoor surgery, the thread ball for backdoor surgery must be removed from the conjunctival side.

The thread ball on the conjunctival side cannot be approached with a normal incisional double fold,
Complete removal of the backing threads is not possible. Partial removal other than the thread ball is possible.

If there are multiple thread balls in a single thread, it is necessary to find all the thread balls and remove them from the surrounding tissue, which makes complete removal more time-consuming. Since the threads themselves are easy to find, there is a higher chance of complete removal compared to backstitching.

In addition to this, complete removal is more difficult in cases where a patient has undergone multiple implantation procedures or where many years have passed since the implantation procedure was performed.

Obsession with complete removal of the implantation threads will increase the surgery time, cause unnecessary scarring of the tissue, and increase swelling.
In my case, I perform surgery based on the policy of "removing as much as possible of the implantation threads that are exposed in the process of creating a double fold.

I'm worried that the implantable threads might do something wrong in the future.

In cases where the thread ball is transparent, it is easy to find the thread ball, so please be assured that it can be extracted almost 100% of the time.

Although it is unlikely that any residual threads will cause future problems, it would be a good idea to choose a simple method that is easy to remove, even when undergoing implantation.
