As the letters indicate, the term refers to whether the eyeballs appear to be retracted or protruding.

Medically, the treatment may include ocular protrusion, such as in Graves' disease, or ocular depression, such as in orbital floor fractures.

However, what is cosmetically referred to as a "deep eye" or "protruding eye" is a relative expression of how the eye looks within the normal range. There is no clear definition.

You can determine whether you have deep or protruding eyes by whether the black eye of the eyeball is positioned in front of or behind the brow bone or cheekbone. To easily determine this by yourself, try covering your eyes with the palm of your hand. If you cannot touch the bulge of the eyeball with the palm of your hand, it is likely to be a back eye, and if you can touch it, it is likely to be a protruding eye.

Treatment of back and protruding eyes

It is difficult to treat the deep or protruding eyes themselves with cosmetic surgical procedures. Although it is possible to treat such conditions associated with Graves' disease or facial bone fractures, it is not common to treat a deep or protruding eye in a cosmetic surgical procedure. It is more realistic to consider the deep and protruding eyes as individual characteristics and to consider a procedure that is appropriate for each characteristic.

Because the eyelids of the inner eye open and close vertically and the distance between the eyes and eyebrows is close, there is more skin covering the double width line. This makes it difficult to create a wide double fold. If a wider double fold is forced, the eye may look unnatural, as if the double fold line is just below the eyebrows. It is important to create a double fold line that does not look unnatural rather than a wide double fold width. With surgeries such as blepharoplasty, blepharoplasty, and glamorous lines, it can be difficult to make significant changes to the inner eye.

In the protruding eye, the eyelid opens and closes in an arc along the eyeball and the distance between the eye and eyebrow is far, so there is less skin covering the double fold line. This makes it easier to create a wide double fold. Conversely, if a modest double fold is desired, care must be taken in the design or a wider double fold may be created than desired. For procedures such as blepharoplasty, blepharoplasty, and glamorous lines, the protruding eye tends to be easier to create a larger change.

In this patient with a tendency toward a deep eye, the incision line is set 7 mm from the lash line and a 2 mm skin excision is made.

On the other hand, this person with a tendency to protrude also has a 2mm skin excision with the incision line set 7mm from the lash line.

Even with the same line setting, the result will be different for people with deep eyes and those with protruding eyes. Of course, the width of the bifocal fold is also affected by various other factors, such as the amount of excess skin, the habit of raising the eyebrows, and the degree of eye opening. Therefore, you can see that statements such as "I read on the Internet that ⚪︎mm is good" or "In this case, it says ⚪︎mm, so please make it the same" are not accurate.

I have written about the characteristics of deep-set and protruding eyes, but in actual practice, the majority of people have average eyes that are neither, and occasionally there are people with deep-set or protruding eyes that can affect the treatment plan. There is no need for patients to be overly concerned about whether they have deep or protruding eyes.

Suppose a person with deep eyes comes for a consultation with a picture of a person with protruding eyes who wants a wide double-edged eyelid. In that case, the doctor will explain the difference between the two eyes and guide the patient to a policy that is suitable for the inner eye.
