This column is about sagging.
Sagging is a change in the texture of the skin and the subcutaneous tissues such as fat, muscles, and supporting ligaments caused by age-related changes. These changes cause a variety of symptoms.
Let us now look at the symptoms of sagging in different parts of the face.

<Forehead and temples

The skin becomes thinner and less subcutaneous fat is present, resulting in a concavity in the center of the forehead and temples. Lateral wrinkles are seen when the forehead muscles move, such as when the eyebrows are raised.

<Upper eyelid

The skin becomes thinned, resulting in fine wrinkles called "crepe wrinkles. The movement of the orbicularis oculi muscle to close the eyes causes crow's feet-like expression wrinkles on the inner and outer sides of the eyes.
Loosening of the skin is more pronounced on the outside, and the double width tends to narrow on the outside and widen rather than widen on the inside.

<Lower eyelid

As the ligaments and the membrane surrounding the fat relax, the fat around the eyes moves downward, causing the upper eyelids to concave and the lower eyelids to protrude forward, resulting in dark circles under the eyes. This results in a groove-like depression in the cheek area called the "Gorgon lines.

The proportion of fat in the face is said to be 80% from the cheeks down. As aging progresses, the skin loses its firmness and, along with the fat, drops down, unable to resist gravity, revealing the skeletal lines of the face. As a result, the gorgon lines and laugh lines deepen, and a concavity is seen in the lower part of the cheekbones.

<Mouth area

Age-related changes in facial contours are most noticeable on the outer side of the mouth, the lowest part of the face. In particular, the furrows that form downward from the corners of the mouth, called marionette lines, become more prominent as the skin, subcutaneous fat, and fascia droop.

Since the neck has thin skin and little subcutaneous fat, the thinning of the skin and the decrease in its tautness are directly responsible for the sagging sensation. Vertical cord-like bands also appear. With further aging, the longitudinal cord-like skin loosens strongly in the center, a condition known as turkey wrinkles.
